Hong Kong, China Ice Hockey Association(HKCIHA) is pleased to announce the HKCIHA Registered Ice Hockey Club application.
Club Applicants must complete this form and provide the relevant supporting documentation in order to be considered as a valid HKCIHA Registered Club.
Clubs must pay a year fee of HK$3,000.

Payment can be made by cheque (make payable to Hong Kong, China Ice Hockey Association Ltd.),No refunds will be given under any circumstances, Please give the application form and cheque with a copy of BR, business/club registration copy and the ID card copy of Person-In-Charge.
