All selected athletes will receive emails with payment instructions.

• All selected players must sign the Code of Conduct & Waiver Form.
• Players under 18 require signed under-age waiver by parents.

• 所有代表球員必須簽署球員守則及免責聲明。
• 未滿十八歲的球員，必須由父母簽署未成年免責聲明。

All players must submit a video for selection.

1. Video / Information Submission: Upload game video(s) with a minimum duration of 5 minutes and a maximum of 10 minutes (video(s) shorter than 5 minutes may not be reviewed, video must be the most recent 6 months) This must be done by completing the Google Form https://forms.gle/TDp7UxMBzPLqRYiu6 before the application deadline.
2. Online Registration: Make sure to complete the online registration form as well.

Important Note: Failure to submit the required video(s) and information by the deadline will be treated as withdrawal.

1. 影片提交：請上傳至少5分鐘、最多 10 分鐘的比賽影片（少於5分鐘的影片可能不予審核，且影片須為最近六個月內拍攝），並通過填寫Google表單完成此步驟： https://forms.gle/TDp7UxMBzPLqRYiu6。請在報名截止日期前提交。
2. 網上報名：完成網上報名表格。

重要提示：未在截止日期前提交所需影片及資料將被視為撤回申請。

球員甄選由中國香港冰球總會全權負責。
