• Own full gear required
• Able to skate & basic ice-hockey experience
• *All Applicants must be Registered Player of the HKCIHA. Applicants who are not HKCIHA registered players will not be eligible to take part in the ballot.

HKD $2,300 for 10 practices (1.5 h each)
Quota: 20 pax (3–5 on wait-list)
No refund except non-selection or program cancellation.

港幣$2,300 / 10堂，每堂1.5小時
名額：20人（候補3–5人）
除非球員未入選或培訓計劃取消，任何情況下均不退款。

Every applicant has to pay immediately after registration. It’ll be refunded to you if your kid is not selected to the training. Participants who have not made the payment will not be eligible to take part in the ballot.

Payments will only be accepted in cheques, made payable to: Hong Kong, China Ice Hockey Association Limited. Please DO NOT make payment by bank transfer. Please send the check directly to our branch office by post or hand it in person to our branch office.

每位申請者必須在報名後立即付款。如果球員未能入選，款項將會退還。未付款的申請者將沒有資格參加抽籤。

付款僅接受支票，抬頭請寫：中國香港冰球總會有限公司。請勿透過銀行轉帳付款。請將支票直接郵寄或親自遞交至我們的九龍灣辦公室。

• All selected players must sign the Code of Conduct & Waiver Form.
• Under-age players require a parental-signed under-age waiver.

• 所有代表球員必須簽署球員守則及免責聲明。
• 未滿十八歲的球員須由父母簽署未成年免責聲明。

Player selection will be made by HKCIHA.
This training program is subvented by the Leisure and Cultural Services Department (LCSD).

球員甄選由中國香港冰球總會全權負責。
此培訓計劃由康樂及文化事務署（LCSD）資助。
